Clackamas County Primary Care Services run three integrated health care clinics that combine medical, dental, and mental health services under one roof.  These clinics are full service medical facilities that can help you with a wide range of medical, dental, and mental health needs.  They feature excellent staff, and highly trained doctors and nurses.  Our clinics use the latest in medical practices that focus on each patient as an individual, and work to take care of the whole person.

Sunnyside Health and Wellness Center

The Sunnyside Health and Wellness Center is a full service medical clinic. We offer affordable, high quality health care services for the entire family, as well as dental care (Summer 2012). The clinic opened on February 13th, 2012 and is now accepting new patients.

Integrated behavioral health services provided at Sunnyside Health and Wellness Center are for those primary care patients established at that clinic. We do not have the capacity to serve behavioral health patients who are established with another primary care provider. For patients who are not established, or do not want to be, please call the MHO at 503-742-5335 and you will be referred to an appropriate community provider.

Gladstone Clinic at
The Gladstone Center for Children and Families

The Gladstone clinic serves children and adults with full service primary care.  The clinic features dental services on a regular basis through a dental van, please call for the current schedule.  Mental health services for children are provided onsite; adult mental health services are available through referral.
