Self-Sufficiency Services

Self-Sufficiency Case Management

Self-sufficiency case management services are available for a limited number of low-income families with children, who are residents of Clackamas County. Families may live at the emergency church shelters in Lake Oswego and Aurora or at the Jannsen Street Transitional Housing Project; there are also participating families in the Six Month Rent program and families referred by the community.

A case manager conducts intake assessment to establish needs and set goals to achieve self-sufficiency. The case manager also provides information and referral, advocacy, and crisis intervention services.

Agencies can call and advocate for a family to be considered for case management. This program does not provide services to persons with drug or alcohol addictions, persons over 55, or persons with serious or persistent mental illness.

Families placed in the emergency church shelters are referred by Annie Ross House. Families placed at Jannsen Street are nominated by Annie Ross House and Clackamas Women's Services. Families participating in the Six Month Rent program are nominated by the community and evaluated by a panel composed of representatives from a variety of agencies.

Health Equipment Loan Program

This program distributes preowned, reusable health equipment to senior citizens and persons who are temporarily disabled. Medical equipment such as walkers, wheelchairs, commodes, and beds are available at affordable fixed fees, and distributed from five convenient storage sites in the county. Loan sites: Canby, Estacada, Hoodland, Molalla and the main site is at Senior Citizens Council office in Oregon City.

This program also provides information, consultation, and referral on appropriate health equipment resources to meet clients' personal situations, including information on cost and availability.

Home Safety Checkup

Trained volunteer Home Safety Inspectors visit the homes of elderly and persons with disabilities in Clackamas County. Volunteers help residents evaluate their homes in relation to accident, fire, crime and fraud prevention. A new smoke alarm is installed at no charge.

Weatherization is also discussed. This includes carbon monoxide detection, lack of heat and excessive moisture problems. Follow-up services through the county weatherization and LIEAP programs are available to those who qualify.

In Home Care Services

In-home services are available for Clackamas County residents who are aged 60 years and older or 18 years and older who have disabilities. In-home services may include housekeeping, meal preparation, grocery shopping, laundry, companionship, personal care, respite care for caregivers, and adult day care. To qualify, persons must be at-risk of institutionalization without the assistance of in-home services. There are two programs which provide in-home services: Oregon Project Independence (OPI) and Medicaid. A sliding fee scale is charged for Oregon Project Independence (OPI) services. Medicaid-eligible clients also may be required to pay for a portion of the cost of the services.

For information and application call CCSS, (503) 655-8640 and ask for the Screener. Initial information is taken by the Screener. After this assessment, if the person appears eligible for services, an appointment will be scheduled.

The program provides referrals for assistance with many household tasks including cleaning, meal preparation, bathing, and personal care. It serves private-pay as well as Medicaid-eligble clients. The program offers an "In-Home Care Workbook" to help clients in the interviewing and hiring process. Providers are interviewed and screened, references are called, and a criminal check is completed before the provider is referred to the client. Working hours, duties, and the rate of pay are negotiated directly by the client and worker. There is a $35 referral fee for private pay clients with the ability to pay. Wages paid directly by the client to worker.

Meals Program

Clackamas County Senior Centers serve meals in group settings and also deliver meals to older adults who cannot come to the centers. Centers always need volunteers to serve and deliver meals.
